You light up my life... eventually

How would you interpret that warning? Are totally enclosed fixtures okay?

Not sure, but it sounds like it is not acceptible for canister type lighting.

I've used these in enclosed overhead fixtures for years without incident. I suspect it's overhead fixtures that are enclosed and recessed in the ceiling they want you to avoid.

Hmmm, yes, it does seem to say I'm using it in an okay method. I guess it was just a dud bulb, or perhaps it got too hot anyway. I'm not using a recessed canister, but the "shade" is a closed glass bulb.

You know not to throw it in the regular garbage, right? Because there's a minute amount of mercury in the bulb somewhere. I have no idea where/how you are to dispose of it... maybe Google will tell us.

Yep. I think Home Depot recycles them. It's sitting on my (very cluttered) desk right now, waiting to be taken care of. ;)
